How to Get Started in 3 Easy Steps

Step 1: Complete Your Participant Details
Start by filling out your personal information, including your full name, NDIS number, date of birth, and contact details. Add your plan details such as start and end dates, funding for Support Coordination (if known), and the type of coordination you need — either Level 2 (Support Coordination) or Level 3 (Specialist Support Coordination).


Step 2: Share Your Goals and Preferences
Tell us what you want to achieve with your NDIS plan. Include any preferred providers, supports, or goals you’d like help with. If someone will act on your behalf (like a nominee, guardian, or carer), include their details too.


Step 3: Review, Consent & Submit
Go over the service agreement and consent sections, tick the relevant boxes, and sign. Once done, return your completed form to your chosen Support Coordination provider.
